AIRWAY MANAGEMENT OF A VALLECULAR CYST EXCISION-A NIGHTMARE TO ANAESTHETISTS

Vallecular cysts are mucus retention cyst of salivary gland and might be a cause ofobstructing the laryngeal inlet posing threat to anaesthetist for airway management(1).We present management an adult patient with difficult large vallecular cyst. A16 year old female posted robotic assisted excision under general anaesthesia. After induction anaesthesia, laryngoscopy was attempted C-mac blade size-3 no structures were seen except epiglottis. We unable lift epiglottis visualise vocal cords since in space between base tongue An immediate plan fibreoptic bronchoscopy guided intubation planned. In meantime, being ventilated by bag mask ventilation. placing Fibreopticbronchoscope trachea just above carina, railroading endotracheal tube difficult.Hence,we decided introduce Video laryngoscope passage which fibreopticbronchoscope. The main aim is prevent damage ensure safe placement adequate ventilation potential related complications.
